I authorize Northeast Health System(NHS), the parent company of Beverly, Addison Gilbert and Bayridge hospitals, and Beverly Hospital at Danvers to use my story submission in marketing, public relations, collateral and public awareness materials, including posting on www.beverlyhospital.org and/or addisongilbert.org/ I understand that my submission may be edited to adhere to word count guidelines inherent to specific projects.

I understand that there will be no monetary compensation for submission and usage of my story. No personal information listed above with the exception of the story itself and my town will be published. Publication of my story is upto the sole discretion of NHS.
