Addison Gilbert Primary Care is an affiliate of Beverly Hospital
located on the 4th floor of Addison Gilbert Hospital in
Gloucester. Caring about every aspect of our patients' lives
is central to our primary care philosophy.
Dr. Candace L. Thompson has joined Addison Gilbert
Primary Care. Dr. Thompson has practiced on Cape Ann for the
past 10 years. During that time she has become known for
providing her patients with high quality, personalized health
care. By joining Addison Gilbert Primary Care, she is
able to provide her patients convenient access to the many
diagnostic, specialty and rehabilitation services within the
hospital.

For your convenience, our patients may be seen Saturdays for
same day sick visits at Lahey Outpatient Center, Danvers (formerly
Beverly Hospital at Danvers) 480 Maple Street. A same day
sick visit can be scheduled by calling 978.304.8354 on Saturdays
between the hours of 8 a.m. to 12 p.m. Please note this is
for Saturday appointments only.
Prior to your first appointment with us, please print this patient profile
and registration, complete it and bring it with you to your
next appointment. Please complete the form whenever there is a
change to your personal and/or insurance information and bring it
with you to your next appointment.
Onsite Services
We can accommodate same-day sick appointments, and see all new
patients within one week of their initial call. Lab and x-ray
services and specialists at Addison Gilbert Hospital are readily
available. Our electronic medical records ensure timeliness,
security and accessibility of crucial patient data, as well as
direct transfer of prescriptions from physician to pharmacist for
greater accuracy. The office is handicapped accessible.

request.
Major Insurances Accepted
We accept most major insurance plans.
However, we do not accept Tufts Medicare Preferred and Neighborhood
Health Plan.
