To do it well, we take the time to get to know our patients, their individual lifestyles, concerns, medical histories. In this way, we are able to prevent, diagnose and treat illness most effectively.

OFFICE HOURS

Monday, Wednesday, Thursday 7:30 a.m. - 7:00 p.m.
Tuesday & Friday 7:30 a.m. - 5:00 p.m.

 

Saturday 8:00 a.m. - 12:00 noon at Beverly Hospital at Danvers. Patients of this practice may make a Saturday appointment to be seen by the provider on call at Beverly Hospital at Danvers. To be scheduled for a Saturday appointment, please call this practice at 978.927.1859.

Prior to your first appointment with us, please print this patient profile and registration, complete it and bring it with you to your next appointment. Please complete the form whenever there is a change to your personal and/or insurance information and bring it with you to your next appointment.

 

 

OFF-HOURS' COVERAGE

On-call coverage either physician or nurse practictioner. Always available by phone but not necessarily from the Cummings Center office.

 

 

APPOINTMENT REQUESTS

Contact office. No email option. Same day sick visits.

 

 

REFERRALS

Referrals are handled through our Central Referrals Office.

 

 

PRESCRIPTION REFILLS

Refills are processed within 24-48 hours of request.

MAJOR INSURANCES ACCEPTED

We accept most major insurance plans. However, not accepting First Security or new Mass Health patients. Please call ahead to determine if your insurance is accepted.
