If you wish to be removed from our mailing list, please complete the General Inquiry form below and choose General Inquiry in the Attention field. In the Inquiry field, type "Please remove the following individual(s) from your mailing list." If you are requesting that several individuals at a particular address are to be removed, you must list the first name, middle initial (optional) and last name of those individuals to be assured that all individuals at the given address are removed.
