Coordinated Care and Health Management for Seniors 

Lahey Health Continuing Care provides high quality, client-centered services that assist members of our community and their families with a unique continuum of healthcare services on the North Shore.

Seniors benefit by receiving care from an integrated healthcare system, where services are coordinated and convenient, and communication takes place between multiple providers; services and coordinated care management are readily available at many different levels. It is not unusual for a patient/client/resident of a Lahey Health Continuing Care program to simultaneously utilize multiple programs and services within Lahey Health, including services within acute care and behavioral health. Programs have expanded to meet the increasing and diverse healthcare needs of the rapidly growing senior population.

The Continuing Care continuum is comprised of the following programs --


Lahey Health at Home

  • Private Duty Home Care
    Providing a wide range of at home services for a wide range of needs by licensed professionals to clients in private homes, assisted living facilities, skilled nursing centers and hospitals. Private duty/fee for service program.

  • Visiting Home Care
    Certified home health services for medically necessary needs, requested by your physician, Visiting Health Care includes visiting nursing services, rehabilitation therapy services, medical social work, nutrition therapy services, and more. This is a Medicare-insured covered program.

  • Monitoring Services
    Offering systems that can help maintain independence, peace of mind and security at home for individuals and their families. The Personal Emergency Response Service provides access to emergency services 24 hours a day, at the push of a button, for anyone who lives alone, is at risk for falls, or who is recuperating from an illness or surgery. The Personal Medication Dispensing System can help anyone who has trouble remembering, organizing and taking their medications. This system provides verbal reminders and dispenses oral medications. Beverly Hospital Lifeline has been serving residents of the North Shore for over 25 years.

Spectrum Adult Day Health

This adult day health program provides a specialized and supportive environment for people with Alzheimer's disease and related memory disorders. The only day program in the region recognized by the Alzheimer's Association of Eastern Massachusetts for its commitment to staff training and support. Spectrum's sensitive, caring, multidisciplinary team is uniquely qualified to care for individuals with cognitive impairments. This cost-effective option for caregivers can enable those with Alzheimer's disease or other types of memory loss to maintain independence and remain in their own homes, giving families well deserved respite and peace of mind that their loved ones are in a safe and uplifting environment.     The Herrick House, an assisted living residence, offers seniors independence and companionship in a supportive and safe environment. Located on Beverly Hospital's upper campus and surrounded by meticulously landscaped grounds, flower gardens and walking paths. The closely knit senior living community features 88 private apartments with many beautifully appointed common areas for residents to enjoy. The experienced staff includes 24-hour nurses, resident care aides, a social worker and a recreation director, all of whom work together to meet the needs of each individual resident. Offering a program specially designed for residents who suffer from cognitive impairment, including those who have been diagnosed with Alzheimer's disease or other memory disorders.
  • Seacoast Nursing and Rehabilitation Center

    Located on the campus of Addison Gilbert Hospital in Gloucester, Seacoast Nursing and Rehabilitation Center offers a friendly and elegant setting with an extensive program of short-term rehabilitation and skilled nursing care. This short-term, 142-bed rehabilitation program also offers compassionate care for residents with Alzheimer's disease and other memory disorders, provided by a certified staff in a secure and therapeutic environment.

  • Ledgewood Rehabilitation and Skilled Nursing Center

    Ledgewood is located on the Beverly Hospital campus and offers superior physician-directed, short-term rehabilitation, complex medical care and skilled nursing services. This is a 123-bed rehabilitation facility with programs designed to ensure patients receive proper care and therapy needed to make a safe transition home.
