Ledgewood is a 123-bed facility providing advanced, patient centered, physician-directed inpatient rehabilitation services and nursing care. The unique hospital campus location provides for a smooth transition from the hospital as well as immediate access to emergency and acute care services and physicians, when necessary. At Ledgewood, continuous performance improvement is driven by patient and family satisfaction and clinical outcomes.

Ledgewood is accredited by the Joint Commission and is affiliated with Beverly Hospital and Kindred Healthcare, providing unparalleled access to resources in both acute care and rehabilitation therapy. Interdisciplinary Team

At Ledgewood, every patient's care is coordinated by a team of clinical specialists including a physician, nurse practitioner, primary care nurse, physical therapist, occupational therapist, speech therapist, social worker, and dietician. While the clinical staff coordinate the day to day care, the patient and family are truly the core of the team and serve to drive the treatment plan, rehabilitation goals and pace of recovery.

Since everyone's pace of recovery is different, the amount of therapy a patient receives each day is based upon their individual needs. In addition to traditional modalities, Ledgewood utilizes state-of-the-art rehabilitation equipment and modalities, including:

• Ultrasound treatment for soft tissue management
• Electrical stimulation for pain management and muscle strengthening
• Short Wave Diathermy for edema management

The rehabilitation gymnasium at Ledgewood provides the ideal environment for therapy. The spacious, sun filled gym provides ample private space for each patient to receive therapy, while other areas of the gym are ideal for group rehabilitation exercise sessions. The gym also houses a training kitchen, transitional bedroom suite and restroom. Together, these areas provide a simulated home environment, ideal for patients to practice being independent while under the supervision of a therapist.

The short-term inpatient rehabilitation program at Ledgewood is designed to ensure that patients receive the proper care and therapy enabling them to return to their highest possible level of functioning before making a safe transition to their home.

Ensuring Your Comfort

Ledgewood's short-term rehabilitation unit offers comfortably furnished private and semi-private accommodations equipped with complimentary cable television and telephones. Beautifully adorned common areas throughout the first floor provide additional options for visiting with friends and family or enjoying some quiet leisure time. During a stay on the short-term rehabilitation unit patients can expect to be surrounded by others with similar rehabilitation needs.

Because proper nourishment is critical to the recovery process, patients may select from a varied menu of appetizing dishes exquisitely prepared to accommodate individual preference and nutritional needs. We strive to make each patient's stay at Ledgewood as comfortable as possible and encourage patients to let us know of any special requests they may have.

A Safe Transition Home

A safe transition to your home environment is as important as a successful recovery. To that end, the clinical care team works together to address all of the aspects of planning for a patient's discharge.

When nearing completion of the rehabilitation program, the transition program at Ledgewood provides an opportunity for patients to practice being independent in a simulated home environment. Through this "supervised independence" patients gain confidence in their restored level of independence and their ability to return to their previous environment and lifestyle. At this time, the therapist will also assess the safety of the patient's home environment and often make recommendations for modifications to the environment and adaptive equipment as appropriate. Patients may also receive home management skills training to provide further assurance of their safety at home.

In many cases, in-home healthcare services are required for a brief period following discharge. Ledgewood will coordinate these in-home services (i.e., visiting nurse or physical therapist) to assist with the transition and provide additional home health aid for the patient. Ledgewood is affiliated with Northeast HomeCare (a member of the Beverly Hospital's healthcare continuum), a quality homecare agency that provides this type of follow up in-home care.

In some cases, patients are ready for discharge from Ledgewood, but are unsure if they want to return to living independently in their own home. In these cases, Ledgewood can help to coordinate a temporary stay in an assisted living facility. Ledgewood is affiliated with The Herrick House assisted living community (a member of the Beverly Hospital's healthcare continuum), which sits directly across the parking lot from Ledgewood.
