SENIOR HEALTH
Senior Health Overview

The Senior Health division of Northeast Health System provides a unique continuum of healthcare services to seniors on the North Shore. Programs have expanded to meet the increasing and diverse healthcare needs of the rapidly growing senior population in our community.
Seniors benefit by receiving care from an integrated healthcare system, where services are coordinated and convenient, and communication takes place between multiple providers; services and coordinated care management are readily available at many different levels. It is not unusual for a patient/client/resident of a Northeast Senior Health program to simultaneously utilize multiple programs and services within Northeast Health System, including services within acute care and behavioral health.
The Senior Health continuum is comprised of the following programs --
Medically-Assisted Housing
Nursing and Rehabilitation Facilities
Home Care
Personal Emergency Response
Educational Programs
Community Care Management
Outpatient Medical & Behavioral Health Services
Day Program for People Living with Alzheimer's Disease
