Joint Replacement Personal Care Plan

At Beverly Hospital, our focus is on you as a whole person, not just on the joint that is being replaced. When you have joint replacement, we will develop a Personalized Care Plan to take care of you every step of the way.

From the time your joint replacement surgery is scheduled to your safe arrival back home, the Beverly Hospital orthopedics team provides you with a personalized experience from start to finish that will exceed your expectations at every step.

Here is what your Personal Care Plan will entail:

Step 1: Joint Learning Class

The first step in delivering  your customized care experience is the preoperative education class that provides valuable and detailed information about what to expect before, during and after your hospital admission. During this class, you will receive the "Preparing for Your Joint Replacement Surgery" patient guide, which you can reference as you prepare and recover from your surgery. Attending this class is an important part of achieving your best results.

You also will be asked to have a coach to help you through your surgery. Your coach could be a family member or friend who will support you through your joint replacement and recovery. Your coach is warmly welcomed by our staff and can help you feel comfortable with your procedure and feel safe going home.

Step 2: "Prehab" Program

A few weeks prior to surgery, we offer and strongly encourage patients attend "Pre-Hab." This   complimentary pre-operative rehabilitation session prepares you for postoperative rehabilitation care (rehab), which is an essential part of your recovery. You will receive a complimentary review and one-on-one education of your at-home, postoperative recovery plan.

The program also will:

  • Increase your conditioning prior to surgery
  • Increase your upper and lower body strength
  • Teach and help you practice the exercises you'll need after your surgery
  • Help you use any assistive devices, such as crutches, for your activities of daily living
  • Review the things you can do to stay healthy after your joint replacement

Step 3: Coordination of Care Meetings

The week prior to your arrival, our staff will review your information to identify any special needs to keep you comfortable and ensure timely discharge and ease of recovery. Do you need an interpreter? Or a longer bed because you are tall? Do you use a CPAP machine?  All of your needs are reviewed to make sure that not only are you prepared, but our staff is prepared as well.

Step 4: Multidisciplinary Rounds and Nursing Shift Transitions

Your care team including your nurses, therapists and case manager meet daily during your stay to ensure alignment in the delivery of your personalized plan of care.

Step 5: Discharge Plan

Once your care experience is complete at Beverly Hospital, we ensure that your personalized care plan will be carried out after discharge. As part of the joint replacement team, a dedicated care manager works directly with you and your family to identify the ideal care destination and facilitate a smooth transition of care.

After Your Joint Replacement Surgery

The Beverly Hospital Center for Rehabilitation and Sports Medicine offers top-quality inpatient and outpatient physical and occupational therapy for people who have had a joint replacement.

Beverly Hospital is also affiliated with two premier facilities for short-term orthopedic rehabilitation - Ledgewood Rehabilitation and Nursing Center on the campus of Beverly Hospital and Seacoast Nursing and Rehabilitation Center on the campus of Addison Gilbert Hospital in Gloucester.
