Our Certified Breast Patient Navigators are trained to guide patients and their families through every step of their care. They are there to support and educate patients when needed.
They establishes close relationships with patients who are at different stages of screening and care - from those who are worried about their breast health to those with benign breast disease to those who have been diagnosed with breast cancer - they are available to help. The Certified Breast Patient Navigators helps patients feel secure that their care is well coordinated and that they are prepared for the next step.
- Guide the individual through a free, personalized Breast Cancer Risk Assessment in a one-on-one environment
- Listen to, discuss and address specific concerns about breast health
- Follow up with individuals whose screening mammograms indicate the need for further evaluation
- Ensure that the channels of communication - among the team, with the patient and family, and with the patient's primary care provider - are open and effective
- Help in any other way possible
