If you are caring for an elderly person with multiple needs, you
do not need to go to multiple locations. Being an adult caregiver
is hard work. Monitoring prescription drugs, coordinating
appointments, providing transportation -- the tasks never seem to
end. Fortunately, there is one source where you can access all the
services you need to care for loved ones with complex medical needs
-- the Center for Healthy Aging at Addison Gilbert Hospital in
Gloucester and at Lahey Outpatient Center, Danvers.
The Center for Healthy Aging is a unique multidisciplinary,
outpatient medical clinic focused on meeting the needs of adults of
all ages, families, caregivers. The Center provides
specialty consultations to geriatric patients and is focused
on meeting the healthcare needs of seniors by providing the highest
level of care to patients by utilizing the best practices in the
field of geriatrics.

- Clinical Nurse Specialists
- Licensed Independent Clinical Social Workers

Specialty Consultations
Occasionally, there are patients who suffer from multiple
medical, psychiatric and social problems. Our Specialty
Consultation Clinic at the Center for Healthy Aging ensures that
these individuals receive comprehensive evaluations from a team of
specialists who, in consult with the patient's primary care
physician, arrange for appropriate services. Those services
can include:
Behavioral Health
Psychological assessment and counseling are performed to
identify any underlying issues. Through the Behavioral Health
consultation, the Center offers diagnostic testing, medication
management, individual therapy, family therapy, and referrals t
ohome-based medical services and agencies.
Memory Loss Assessment
A complete medical evaluation, psychological assessment and
neuropsychological test are performed as needed to assess your
loved one's needs and develop a support plan for him or her, as
well as for you and your family.
Balance and Falls Risk Assessment
Through a comprehensive evaluation, potential causes and risk
factors can be addressed to assist in reducing the risk of
recurrent falls. If your loved one has experienced more than
two falls in a six-month period, or experienced a change in
balance or walking ability, he or she should be evaluated to
determine any underlying cause or causes.
Driving Assessment
Through an objective medical evaluation, the Center can
assist in determining if you or your loved one is able to continue
driving safely. The objective data and advice provided can
assist you and your family in making decisions about
driving.
Accessing Our Services
Referrals to the Center for Healthy Aging come from primary care
and specialty care physicians, eleder care agencies, hospitals, and
assisted living facilities. Services are coved by Medicare
