The secure pre-registration form is only for those patients who have an appointment at one of following hospitals-- Addison Gilbert Hospital; Beverly Hospital; and Lahey Outpatient Center, Danvers. We have designed this online pre-registration form to allow you to pre-register from home for your upcoming appointment. Please complete this form at least 24 hours prior to your appointment. This gives our staff the time needed to process your pre-registration information.
