Plan for Your Stay

You may have lots of questions before a hospital stay or procedure. We can help you plan for your care. Learn what to expect before, during and after procedures during a hospital stay or same-day visit.

Health Care Proxy

A health care proxy allows you to designate someone to make health care decisions for you if you become unable to make choices on your own. This type of advance directive is legally recognized.

The health care proxy only takes effect if your doctor decides you are unable to make or communicate your decisions. Once you recover, the proxy is no longer in place. You can specify the decisions your agent can make for you, such as giving permission to:

  • Administer medicine or cardiopulmonary resuscitation (CPR)
  • Perform surgery
  • Use or to stop life support machines

Completing a health care proxy lets you avoid uncertainty around difficult questions. You can change or revoke your health care proxy at any time.

If you have questions, contact Social Work Services.
